Urban Health Plan
Urban Health Plan provides school-based health services right here at PS 75. Our program ensures that students have access to quality, comprehensive care without having to leave school.
Services Available
- Primary health care and wellness visits
- Physicals and immunizations
- Prescriptions (filled at Urban Health offices)
- Medication administration by the school nurse (with parent consent)
- Dental care by appointment
How to Enroll
Students cannot be seen without a completed consent and registration packet. Applications are available in the:
- Nurse’s Office
- Main Office
- School Lobby

- Parents can schedule appointments directly.
- Dental services are also available by appointment.
